Early Signs of Toenail Fungus and What Causes Them
Toenail fungus, also known as Onychomycosis or Tinea Unguium, often begins with subtle changes such as yellowing, thickening, or brittleness of the nails. This condition is commonly caused by fungi like Trichophyton rubrum and Trichophyton interdigitale, which thrive in warm, moist environments. It tends to affect the big or little toenails and becomes more common with age or following other fungal infections like athlete’s foot (Tinea Pedis).
Early symptoms may seem mild but can worsen over time if left untreated, potentially leading to discomfort and complications. Recognising the causes and early signs of toenail fungus is important for effective management and prevention of further spread. Understanding these factors helps you take timely action to protect your foot health and maintain strong, healthy nails.
What are the Common Causes of Fungal Toenail?
Toenail fungus often starts small but can become more troublesome if conditions favour its growth. Several factors increase the likelihood of infection, making it easier for the fungus to thrive and spread.
Below are some of the most common causes linked to fungal toenails:
- Warm and Damp Environments
- Poor Foot Hygiene & Excessive Sweat
- Wet Weather Season
- Nail Trauma
- Athlete’s Foot
- Weakened Immune System

What is a Fungal Toenail?
A fungal toenail is also known as onychomycosis. It is a common condition where the toenail becomes infected by fungi, typically dermatophytes. This infection usually begins when fungi enter the nail through small cracks or breaks in the skin around the nail, especially in warm, moist environments. Over time, the affected nail may become discoloured (often yellow, white, or brown), thickened, brittle, and crumbly. In some cases, the nail can distort in shape or lift away from the nail bed.
While it may seem like a cosmetic issue at first, fungal toenail infections can worsen if left untreated, spreading to other nails and causing discomfort or pain. For individuals with diabetes or poor circulation, the condition can lead to more serious complications. East Coast Podiatry emphasises early diagnosis and treatment using methods such as nail debridement, topical or oral antifungals, and advanced options like photodynamic antimicrobial therapy (PACT) to effectively manage and eliminate the infection.

Methods to Relieve Discomfort and Promote Well-being
Depending on the severity of the infection, different approaches can be used. Below are some available options:

Toenail Debridement
This procedure involves carefully thinning the thickened toenail to enhance the effectiveness of other treatments. It is often performed alongside other care methods to improve outcomes.

Special Light Therapy
Known as Photodynamic Antimicrobial Therapy (PACT), this treatment uses a special photosensitising gel combined with light to specifically target and reduce fungal infection in the nails.

Toenail Removal
For severe or persistent infections, removing the affected toenail through a minor procedure under local anaesthesia may be recommended. This allows a healthy new nail to grow in its place, supporting long-term recovery.

What are the Common Symptoms of Fungal Toenail?
Understanding the Signs for Early Detection
Fungal toenail infections can cause noticeable changes to the appearance and texture of your nails. Recognising these symptoms early is important for effective treatment and preventing the infection from worsening or spreading.

Common signs include:
- Discolouration - Nails may turn yellow, brown, or white as the infection develops.
- Thickening of the Nail - Infected nails often become thicker and harder than usual.
- Brittle or Crumbly Nails - Nails may become fragile, breaking or crumbling easily.
- Distorted Nail Shape - The natural shape of the nail can become irregular or misshapen.
- Separation from the Nail Bed - The nail may lift or detach from the skin underneath.
- Foul Odour - A noticeable unpleasant smell can sometimes be present.

Our 6-year-old son had very severe in-toeing when we first visited Dr. Michael in October 2024. We’re incredibly thankful for his expertise—after just 8 months of treatment, our son is now very close to having an ideal foot structure.
What we appreciated most was the all-in-one pricing—it covered everything from braces to follow-ups, making the treatment both convenient and great value for money. The clinic’s location in Orchard is also a big plus for accessibility. Highly recommend Dr. Michael, especially for concerned parents dealing with pediatric foot issues!

Fungal Toenail Frequently Asked Questions (FAQs)
What causes toenail fungus?
Toenail fungus is caused by fungi that thrive in warm, moist environments. You can contract these fungi through direct skin contact with an infected person or by walking barefoot in communal areas such as pools and locker rooms. Poor foot hygiene and wearing tight or non-breathable shoes can also increase the risk by trapping moisture around the nails.
How can I treat toenail fungus?
Toenail fungus can be difficult to treat with over-the-counter creams alone, as they require consistent use and may not fully eliminate the infection. More effective treatments include Photodynamic Antimicrobial Therapy (PACT) and toenail debridement. In severe cases, toenail removal (avulsion) may be necessary. Consulting a podiatrist ensures you receive a personalised treatment plan to promote healthy nail regrowth and prevent recurrence.
What are the symptoms of fungal toenail infection?
Early symptoms include changes in nail colour such as yellow, white, or brown discolouration. The nail may thicken, become brittle or crumbly, and develop an irregular shape. In some cases, the nail may separate from the nail bed and emit a foul odour. Recognising these signs early helps prevent complications and supports better treatment outcomes.
What is nail avulsion and when is it needed?
Nail avulsion is a minor procedure involving the removal of the entire toenail to treat severe fungal infections that have not responded to other treatments. Performed under local anaesthesia, it helps eliminate infected tissue and encourages healthy nail regrowth. Proper aftercare is essential to ensure healing and reduce the risk of further infection.
What are the risks of ignoring toenail fungus?
Ignoring toenail fungus can lead to worsening infection, spreading to surrounding skin and other nails. This may cause redness, itching, cracked skin, and discomfort. Untreated infections can also result in thickened nails that may detach from the nail bed, increasing the risk of secondary bacterial infections.
Can toenail fungus be prevented?
Yes, practising good foot hygiene, keeping feet dry, wearing breathable footwear, and avoiding walking barefoot in communal areas can reduce the risk. Regularly changing socks and disinfecting shoes can also help prevent fungal infections.
